About the Role:
As a Social Worker, you will play a vital role in delivering strengths-based assessments and interventions for adults living with frailty and complex needs. Your work will focus on reducing hospital admissions, supporting independence, and enhancing quality of life through early intervention and innovative care planning. You will collaborate closely with residents, carers, community organizations, and multidisciplinary teams to ensure holistic and person-centred support.
Key Responsibilities:
The ideal Social Worker will be registered with Social Work England and possess experience working in adult social care. You will be committed to strength-based, person-centred practices and comfortable working in fast-paced environments. Strong relationship-building skills across health and social care sectors are essential.

Key Responsibilities:
- Deliver assessments and interventions under the Care Act, Mental Capacity Act, and other relevant legislation.
- Work collaboratively with residents, carers, health professionals, and community services.
- Attend and contribute to multidisciplinary team (MDT) meetings.
- Identify and respond effectively to safeguarding concerns, adopting a Making Safeguarding Personal approach.
- Maintain accurate and timely case records using the LAS electronic system.
- Support anticipatory care planning and early intervention initiatives.
- Embody the CARE (Collaborative, Ambitious, Resourceful, Empowering) and iCARE (Innovation, Compassion, Accountability, Respect, Excellence) values.
