You will:
- Build and maintain meaningful relationships with young people, ensuring their voices are heard and their needs are met.
- Support care leavers in developing emotional, social, and financial skills for independent living.
- Coordinate services and encourage young people to access the support available.
- Work with partner agencies and professionals to promote positive outcomes.
- Keep accurate records and contribute to pathway plans and reviews.
- Provide a flexible response during times of crisis, including occasional evenings and weekends.
- Act as an advocate for young people’s rights, promoting equality, safeguarding, and well-being.
You will bring:
- A good standard of education to at least NVQ Level 3 (or equivalent).
- Knowledge of the challenges facing young people leaving care.
- Strong communication skills, with the ability to build trust and rapport.
- Effective organisational and time management skills.
- The ability to work both independently and as part of a team.
- A commitment to equality, diversity, and safeguarding.
- Willingness to travel across the borough and work flexibly, including evenings and weekends.
