Community psychiatric nurse

Community Psychiatric Nurse – Dementia Service – NHS – Telford

Service Care Solutions are currently recruiting for a Community Psychiatric Nurse to join a Dementia Service within the NHS, based in Telford.

• Contract Length: Initially 3 months, with potential for extension
• Hours: Full-time, Monday to Friday, 9am–5pm
• Location: Community-based role

Key Responsibilities:

• Provide specialist mental health support to individuals with dementia in the community
• Conduct assessments and develop person-centred care plans
• Administer prescribed treatments and monitor patient progress
• Liaise with families, carers, and multidisciplinary teams to ensure continuity of care
• Contribute to safeguarding processes and attend relevant meetings

Requirements:

• Previous experience in dementia care and community psychiatric nursing
• NMC registration
• Strong clinical, communication, and organisational skills
