Are you a motivated and organised individual looking to make a difference in the healthcare sector? We are currently seeking a Care Navigator to join our Clinical Services and Urgent Care teams in York. As a Care Navigator, you will play a pivotal role in ensuring patients receive high-quality care and a seamless experience.

Key Responsibilities:

• Deliver high-quality administrative support, including appointment booking and referral processing
• Manage patient data accurately and confidentially using SystmOne
• Respond to patient and professional enquiries in a professional and compassionate manner
• Support the coordination of care across services and follow up on patient needs
• Work flexibly across teams to support service delivery

What We’re Looking For:

• Level 2 English and Maths (or equivalent experience)
• Excellent attention to detail and accuracy
• Strong organisational and administrative skills
• Excellent verbal and written communication
• Confident using Microsoft Office applications
• Ability to work independently and as part of a team
• Experience in the NHS or general practice
• Familiarity with SystmOne and referral systems such as Gateway
