Care Navigator - Primary Care

Opportunity for a a Care Navigator to work on a Locum Contract, Based across Yorkshire & Surrounding Areas!
  The role is based within the Primary Care Team, with the role predominately providing administrative support and undertaking tasks and referrals on  clinical systems, alongside clinical coding. You will be the point of contact for patients and healthcare professionals and be able to communicate with patients over the phone in a professional manner creating a positive patient experience. You will be able to accurately record patient details ensuring good attention to detail and take responsibility for any patient enquiry and follow through to conclusion.
